Question about TIWCGJQMDialog

I got sample of Dialog from francesco's post http://www.cgdevtools.com/cgforum/viewtopic.php?f=4&t=1373&start=20
and further modified for my requirements.
Please find attached sample and help me out to pin point the following problems
1. On Pressing OK and Cancel button (in Dialog), dialog doesn't close, which properties of Dialog/button to be set for this purpose?
2. OnVClick of Ok and Cancel button never fires, why?

1. Check the form structure, it's should have the right structure: viewtopic.php?f=6&t=404

2. Check this requirements: viewtopic.php?f=118&t=1366
